What Is a Parental Affidavit for a Child’s Passport?

A parental affidavit is a formal declaration through which consent is given for a passport to be issued to a child. It is used to confirm that no objection is raised by the parent signing the document.

When a parent is living in Australia, the affidavit may need to be prepared in the prescribed format and attested before it can be used for the child’s Indian passport application.

The affidavit supports the application. The child’s passport forms and other required documents must still be submitted separately.

Child in India and Both Parents in Australia

When both parents are in Australia, separate affidavits are required from each parent under the published Australian service checklist.

For the passport application in India, a local guardian must also be appointed. Passport India’s guidance specifies a joint sworn affidavit, attested by the Indian Mission or Post abroad, for this arrangement. Supporting affidavits from two responsible people are also required unless the appointed guardian is a grandparent.

The Australian affidavit requirements and the guardian requirements in India should be reviewed together before documents are signed. A family member is not automatically authorised to submit the application simply because the child is staying with them. Passport India guidance

Documents Required for a Child’s Passport Affidavit

The following documents are listed in the published Australian checklist:

  • A completed and signed Miscellaneous Service application form.
  • A parental affidavit in the prescribed format.
  • Two attested copies of the applying parent’s current passport.
  • One recent photograph measuring 2 × 2 inches.

The affidavit and passport copies must be attested as required. A Justice of the Peace or Notary Public is specified, with Notary Public attestation required for NSW residents.

For Australian citizen parents, the affidavit must be apostilled by the Department of Foreign Affairs and Trade (DFAT) before submission to the application centre. Additional documents may be requested by the Indian High Commission or Consulate. Australian affidavit checklist

The child’s birth certificate, existing passport and any custody or guardianship documents should also be kept ready so that the requirements for the application in India can be checked.

Get the Paperwork Checked Before It Is Sent

An incorrect name, missing signature or unsuitable attestation can lead to further requests and avoidable delays. Before submission, the details on the forms should be checked against the supporting documents.

Particular attention should be given to:

  • The child’s name and date of birth.
  • Each parent’s name and passport details.
  • The correct affidavit format.
  • Signing and witnessing instructions.
  • The appointed guardian’s details, where applicable.
  • Whether original documents must be sent to India.

If documents need to be witnessed, signing instructions should be confirmed before a signature is added.
